[BUG] Claude stops and waits for user input with TODO list

Bug Description

During execution of a task, it just stops. It happens constantly. Note: Bypassing Permissions mode active

I can reproduce this consistently with todo list enabled and repeating the prompt:

use todo list add another item "Hello World" and complete it

After Update Todos, Claude stops and waits for me. No more automatic tasks processing... I got that sinve 2 days ago.

It looks like the problem was fixed after the release of Claude 4, because I haven’t come across it.

Yes, we've made some fixes and this issue has mostly be fixed (there is still a casee where if there are a ton of todo list items claude will still stop and ask for feedback, this is being tracked here: https://github.com/anthropics/claude-code/issues/1632)
