Poisons system python

Description

Even though I have a virtual python environment in the directory I'm running opencode, and it's activated at startup, opencode occasionally "forgets" this and starts installing python libraries with pip directly, going so far as to use the "--break-package-system" option. This should not happen, in my opinion. When I remind opencode to use the .venv, it does so, but will eventually forget again.

Steps to reproduce

Develop and test django applications - I don't know at which stage the .venv was dropped.
