Add nbminer v41.0 -- 100% LHR unlock Some nhml changes are needed .. simply updating the nbminer files is not working

or remove all "lhr" options from extra launch parameters

copying and pasting the new miners worked fine for me didn't change any parameters. there was an older nbminer exe that I deleted which had a version number in it's name
