Cannot see the heroku app

I am signing in the heroku app using the username and password {admin ,admin} but i am unable to sign in .Is the credential correct?

If you run the program locally then it runs.

Even after running the app I am only seeing the waiting list patients and other tables are just not there, Please help!!
