amp-access examples fail with error

What's the issue?

I faced exactly the same issue as described in https://github.com/ampproject/amphtml/issues/2444 But, once I decided to look at examples - I found they are not working

How do we reproduce the issue?

Please try to run sample of amp-access and log in. There is error 500 instead of login window.

What browsers are affected?

I tried Safari and Google Chrome on Mac and Windows - issue is not related to browser.

Which AMP version is affected?

I think this is an amp.dev issue more than an amp-access issue assigning to Paul
