Every new ticket results in a connect ECONNREFUSED 500 error

This is a terrific example but I'm getting a problem that points to util.js:1022:11 Missing stack frames. because this is such a nice and basic example I would like to work out what is going wrong. The ticket is writing to the sqlite3 db - i can see the data in a db browser and the application shows the data when I review tickets.
