Unclear behavior for उष्णिह्

I'm not sure how to derive both उष्णिक् and उष्णिहा. Is अजाद्यतष्ठाप् optional?

No अजाद्यतष्टाप् is not optional in general. But उष्णिह् is itself feminine, so it can be used as it is. And it still gets टाप् because it is included in अजादिगण. Hence both forms are used. There is another argument here. भाष्य says that उष्णिहा is created from उष्णिह (notice full ह) by adding टाप् as this word is अकारान्त.
