Assign milestones by name

As suggested in #59, we should look at being able to assign milestones by name, rather than just ID. Although there is no direct API method to do this, we can retrieve all milestones and then search those to find the ID of the named milestone. This is probably more intuitive than the current ID method alone, so worth the extra request.
