Simplify issue template and add more fields

The current issue template for bugs is significantly better than nothing, but is a bit clunky, with likely redundant fields and poor wording on some. Additionally, it doesn't confirm that the issue is reproducible on a dedicated server, nor does it request the architecture of the server being used (x86 vs x64).
