Currently pending release.

replace confirmation modal window on upload appears briefly and then disappears immediately

Current workaround is to delete the existing file then do the re-upload, but that's clunky. Hoping to look at this soon. :)

Just updated to the most recent version of Jellyfin server. Maybe the issue is on the server end? going to keep an eye on things.
