Update data.yaml - 'real world' examples added based on Companies House data

Added real world examples added based on Companies House data. These additions were based on issues found here: https://www.opensanctions.org/issues/gb_coh_psc/

This pull request should resolve over 600 errors/warnings on jurisdiction, country or nationality.

I also added some subheadings (commented out) to make editing easier. This meant moving a small number of pre-existing entries.

In addition, I removed two duplicates I found.
