Misc doc changes

Besides other documentation changes, this commit ensures the generated HTML doc for HexDocs.pm will become the source of truth for this Elixir library and leverage on latest features of ExDoc.

Hello @kianmeng Sorry for the double work, but I'm still very interested in merging this PR since I'm trying to close all currently pending PRs. Are you able to resolve the conflicts one last time so this can be merged along with the others for the v0.9.3 release?
