Processing instructions gain an extra question mark on EPUB round-trip load/save

If you load and save an EPUB then any processing instructions embedded within the original HTML embed an extra question mark in the saved copy.

For example: <?pagebreak number="1"?>

gets transformed to: <?pagebreak number="1"??>
