Add DDA formulation command line option

Add command line option to choose the whole formulation (like FCD, SO, LDR,
etc.), which will be equivalent to a combination of options -pol, -int and
-scat. However, the possibility to use old options or a combination of new
and old ones should remain.

Since several good alternatives to LDR emerge, special effort should be made to
persuade users to mention the exact formulation of DDA used when they publish 
any
simulated results. This should be included in the manual and announced with the 
next
release.

This should also include a new wiki page with instructions on adding new DDA formulations (as was discussed in #106).
