Is this project still alive?
Since no pull requests accepted anymore, and moreover no active development exists in the commits, I wonder if this project is still alive?
I was hoping that this extension will be like Markdown Extended extension of VSCODE (which supports almost all the markdown-it extensions)
Is there an actively-maintained fork out there?
Kind of. @neilmayhew incorporated some patches into his fork which render the add-on usable in Thunderbird 68+ (but there are still unresolved issues). He released a ready-to-go "community build" so people can just download and install the .xpi file. He welcomes PRs with further improvements in his fork.

See #644 for a working version for TB 78. However, this wasn't done by @adam-p and it seems like it's time to fork the project.
https://github.com/jfx2006/markdown-here looks like the most active one per the link @realstephenzhao shared, but it seems to only target Thunderbird.
