Pulseaudio not detecting changes in output devices

After switching to PuleAudio, I noticed that when I change output devices (headphones, etc.), pulse audio continues playing audio through the previous output device (speakers).

Investigate whether it's possible to trigger a pulseaudio rescan when output devices change.

Restarting pulseaudio is the current workaround on both Mac and Linux.
