Documentation needed to explain permissions and scopes better

This is wrt https://github.com/ably/realtime/blob/develop/common/lib/types/scopeid.js. We currently don't have enough documentation that explains how to set these. Would be helpful to get this out ahead of Channel Lifecycle API release.
