Make an "upsell" UI for micropub

Instead of hiding the response buttons, Monocle should show them, and when clicked, give the user more information about how to enable them.

This pattern is common in freemium apps where the goal is to get the user to subscribe/pay to enable features. In this case, we can use it as an incentive to get people to set up a website that supports Micropub along with their reader.
