[Feature Request] Toggle for default or custom meta templates

Please give us a description of what happened.

There is no easy way to reset the meta templates back to default without resetting all plugin settings.

Please describe what you expected to happen and why.

Add a toggle switch - Default or Custom Show/Hide the title/desc template field [Optional] Show/Hide the other toggles (show in results, date in snippet, meta box) and use defaults for these, as well.

Adding this feature would minimize the amount of repetitive fields and shorten the page making it less overwhelming and easier to identify the post type, taxonomy or other item when making changes.

@hedgefield can you come up with a UI for this that isn't clutter? If we can, I'm inclined to say this is a good idea.

@jdevalk, here is an example of what it could look like.

In Default settings, I thought it would be good if the user would still be able to see what the default settings are. Therefore, the toggles and input fields are still visible, but inactive.
