Add issue template for normal issues

Current issue templates are all off-site links without a clear option for a normal issue, this leads to the issue creation page being slightly confusing. This page does still have an option to create a blank issue at the bottom of the page but this isn't shown the way one will expect and is intentionally smaller.

As reference, the main issue tracker will show issue creation options in green
