add fixes to #597

After some thought, I've fixed most of these with separate commits. Except the autoloader part - can you elaborate on the idea behind those changes? As far as I can tell, they wouldn't silence the audit check you mentioned in #597, and I'm not clear on what threat/issue they're intended to mitigate.
