combine toolbar and titlebar in library

The Library application currently employs a simple toolbar with a few functions. It may look nicer and more balanced to combine the toolbar with the title bar, a pattern becoming increasingly common on desktop applications, and which I believe is supported by current releases of Qt windowing toolkits.

(See also #105. Incidentally, the H keypress has no effect in my instance of the Library application.)

We do that on Mac OS X as it is the platform default, and to do so we have to use platform specific extensions for OS X. I'm not aware of such an extension for other platforms and implementing it is not something we're currently looking into.

I have not done it myself. It appears, however, that the effect is possible in Qt applications, and that resources are available to explain how. It also appears to be feasible without extensions.
