[request] Display configurable header box in-page

Hi and thank you for this great tool. monolith is in fact a data hoarder's dream come true.

My suggestion is the following:

Currently, monolith saves capture metadata to a HTML header comment, as in the example below.

It would be very nice to have an option to display this info on a header box at the top of the captured page and automagically activating the original URL.

Doing so will provide an easy and human readable source attribution, as well as allowing for quick checking said source online.

Something akin to the example below, perhaps with a little background coloring to make it stand out.

Also, please make datetime strings user configurable -- I have no quibble with the present normalized format, but those alphabetic identifiers make it more difficult to read.
