Descriptive Books Wrong Dimension

I'm running a 1.12.2 SMP using the most recent version of Mystcraft along with a few other mods (which I'll list at the bottom). After the server has been running for a while, when using a premade descriptive book it brings me out at the overworld spawn, and once in an incorrect age, instead of the desired age. It is always fixed after a server restart, but eventually breaks again.

I've also noticed that when I teleport back to the overworld the surroundings don't load in clientside, but I think that's an issue with Journeymap.
