dapps/universal-provider-solana: Transaction Issue

I use dapps/universal-provider-solana to send transactions using Trust Wallet QR. However, the transaction was not completed successfully. Although I received a TX hash on the console, it does not exist on Solscan. I have thoroughly checked for any issues but could not find any. Please help me with this matter.
