make the title of a testimonial a combination of various custom fields

Describe the solution you'd like is there anyway to change the default TITLE of the testimonial to be a combination of the data the visitor is entering? Like: Name and Datetime for example: "New Testimonial - July 17, 2020, 10:23 pm"
