enter data > download starts immediately

We want independent fields in the form and only one "Download" button. Only after clicking the "Download" button, a modal box or a new window opens where the visitor enters the required data in the form and then the download starts immediately.

Think this one is for the locking extensions that use data processing? Email Lock, Ninja Forms, Gravity Forms.
