In what kind of world or server did the problem occur?

My multiplayer server that I was running

What went wrong? (Crash logs don't go here)

Since updating my server to Forge 40.2.15, the /scale persist command stopped working altogether. Whenever someone leaves the server, when they come back their scale will be reset and I need to set their scales over and over. I did try to use the /scale persist more than once, and every time it just doesn't work or doesn't register the new scales.
