Cities table issue

Hi,

this is not a technical issue but rather a data-set organization issue.

Data-set of Cities table must be rebuilt entirely since there is no one State-Country correct matching.

This brings an inevitable malfunction of the plugin.

There is also a bug with acf_field_COUNTRY_FIELD Class in v4 and add_action() hook.
