Footnote syntax fixes

Fixed syntax errors for footnote references in fees.md and swaps.md.

For footnotes, [^#] would only create a one-directional linking. To display the return-to-source button, one would need to use [^#]:.

Thanks for the fix! This has been integrated into an existing docs organization overhaul in #490
