Rationalise the handling of user options

Use TOONTALK.DEFAULT_SETTINGS consistently. This enables web pages to have different default settings.

Add new settings to settings page tab (e.g. voice defaults).

Use casing consistently.

And URL options such as speak, balloon, and volume should be passed to links in the same way that translate currently works.
