Freestanding recovery heart not replaced consistently

The attached seed replaces one recovery heart in Ice Cavern with a Goron Tunic. However, upon loading into the room for the first time, the heart is not replaced and does not give the tunic upon collection. Revisiting the room later results in the correct item appearing.

Interestingly, this issue only seems to occur on saves in slot 1.

Tested on RetroArch on Steam Deck.

Repro steps:

1. Start a new game in slot 1.
2. Enter Ice Cavern in its vanilla location.
3. Make your way to the map room.
4. There will be 3 freestanding hearts on the raised platform.
5. Save and reset the game.
6. Return to the map room.
7. Notice the middle heart is now a Goron Tunic.
