Remove the following substitutions

This cleanup task can't be done until the translators are finished.

The following lines will need to be removed from the substitutions files once I have a final version for each language:

Add [shield]1 shield Gain [thorns]1 thorns Add [thorns]1 thorns Recover [heart]1 health Recover [heal]1 health

Do [sword]{amount} Gain [shield]{amount}

And the following lines need to be fixed to use the singular:

inflict [poison]{amount} poison
