Unused maxScale of ArcGisMapServer

Even if server side has been set a maxScale, this is not used and the layer remains visible (badly) even beyond that scale. Perhaps if hideLayerAfterMinScaleDenominator is true, the layer should no longer be visible beyond maxScale.
