Add "report a problem" button to manager

want to make it easy for people to submit bug reports, button should link to the issues page so they can see pinned issues before creating their own submission. would be good to add a troubleshooting wizard to help prevent bug reports from people simply not knowing how to use the mod

It could even auto-fill the report with info such as mod /mc version, a copy of the running code (and any errors), whether or kit other mods are installed (a list?), etc.
