Edit forms from Uploads screen erases userProfile info

Expected behavior

After editing a record from the upload screen I expect that all form information is preserved. We keep the record for { "name": "userProfileId", "value": "83d4f28f6" }, { "name": "tabletUserName", "value": "TableProfile" }

Actual behavior

user profile info is removed
