No information given about how to run build

It would be nice if the docs had an example of running doxygen command. For instance, what does one do with the layout.xml file?

It would also be nice if the header/footer files where a bit more stubbed out instead of being left as an exercise to the reader to fetch code from elsewhere not under source control...
