Add a README.md

Please add a README.md to describe how this works - how one invokes the converter, how one specifies options, how one adds extensions/hooks, etc. For example, how can I extend this to add anchors on section headings, or generate TOC.

thanks for the link @moroz , but that link really needs to be in the README.md here on GitHub, as anyone cloning this repo or viewing it won't see this link buried in this issue. I believe the code.google.com site has been inactive since 2015 so ideally that content should be copied into this project where it can be updated
