Theming of modified organization dialogue is incorrect

Description

The theme of the modified organization dialogue does not match that of the connection dialogue:

Most dialogues in VS are not themed e.g. Tools Options The tabitems are using the correct default light grey colour.

However, the dialog is not inheriting from the correct base class - see https://docs.microsoft.com/en-us/dotnet/api/microsoft.visualstudio.platformui.dialogwindow?view=visualstudiosdk-2017
