Add photos and rating stars

I created a PR: https://github.com/SnowdogApps/SDForms/pull/5 I'm using the stars and photo field for a demo app so I built them into this PR. It's a bit of a pain to deal with personal fork and Cocoapods so if you'd like to consider this for merging that would be great. But if not, I'll know to keep my own fork of this work.

Very nicely written component!
