Fix erroneous invalid attach errors

Fixes #7484 - The current github/smorg preview problem Fixes #7496 - The 2.1.2 insert problem Fixes #7434 - Tweaks the previous fix to the smorg performance problem

This also addresses issues with attachments disappearing/reappearing in dropzone or in your post attachment list during edits, especially after there were post edit errors in posts. No more hijacking attachments from your previous posts.
