bug report template in the example app

How about adding a bug report template in the example app? I got the idea from rnmapbox:

Yes! Sounds like a good idea. We should also add a template for issues and pull requests soon - and maybe even some tests to ensure that we have an issue for a pull request, that all fields in the PR is filled out etc.

I'm closing this issue for now since we now have the issue templates. We also have #147 open for having e2e testing. Please re-open it if needed.
