Jurisdiction enums are not available on case creation form

Bug Description

When creating a case

Reproduction Steps

1. Connect to https://release-international.sormas.netzlink.com/sormas-ui/ using the surveillance officer role for example
2. Open case creation form, the jurisdiction of the user is not prefilled
3. Connect to the same server with a national user role + open case creation form
4. There is no option to choose a facility in the dropdown or to create a case using "other medical facility"

Expected Behavior

• Prefill jurisdiction using user jurisdiction
• Provide option to select other medical facilities + free text
