[Case - Previous Hospitalizations] Display the value from 'Facility name & description' alongside the value from 'Facility'

Problem Description

When the place of stay of a case is set to:

• 'Facility' - Facility category = 'Medical Facility'
• Facility type = 'Hospital'
• Facility = 'Other facility'
• Facility name & description = 'HospRobK' => in the 'Hospitalization' tab of the case, for the section 'Current hospitalization' , the information displayed for the Hospital name is 'Other facility (HospRobK)'.

For the section 'Previous Hospitalizations', in the column 'Hospital', if the same values are set (either by creating a previous hospitalization manually, or by transferring the case), the values listed is only 'Other facility'.

Proposed Change

Display the 'Facility name & description' in the 'Hospital' column as well, for previous hospitalizations, in the same manner that it is displayed for current hospitalizations.
