Revamp phone number inputs

I think we're at the point where having a separate spot for the country code, at a minimum, would be helpful. Not everyone has US phone numbers, and if that's unclear when phone numbers are shown in Nova, it makes the information less useful.

I'm not sure exactly what would be good to implement, but https://github.com/jackocnr/intl-tel-input is what I have in mind.

At a minimum, allow for inclusion of + on the frontend so that things like Square that specifically need that for proper handling can recognize international numbers.
