Instrument repeated

After editing an instrument and saving it locally, it appears twice in my instruments list

I could not reproduce this error when trying with a different, similar instrument

@vkozloff I could not reproduce this error with ADOS Module 3 instrument.

Can I solve this by deleting the termform for one of them?
