Questions are not saved when you add them to a quiz from the question bank

General description

When I am adding questions from the question bank and click back to the questions tab after it saves, the new questions are gone. I tried creating a new quiz and right away adding questions from the bank and some stayed but when I tried to add the 8th questions it disappears again.

Expected behavior

Add new question from question bank to quiz.

Actual behavior

Question does not appear in the list on the quiz you are adding it to.

Steps to reproduce the behavior

Make a quiz, add some questions, click to a different page and come back to the quiz. Then add another question from the question bank, wait for it to save and click a different tab in the quiz settings then go back to your quiz.

PS. When I looked in the console I saw this message:
