Solution file not getting committed to the repo

Facing the issue a lot of times recently. After submitting an accepted solution to Leetcode, only a NOTES.md file is committed to the repo while the actual solution file doesn't get committed. This is happening a lot nowadays in almost every 3 out of 5 problems I solve.
