[Feature] Use placeholder icon on tab for sites that do not have favicon

Describe the issue

Sites that do not have a favicon lose the visual separation that the icon gives, plus the tab text is shifted left:

Request that a generic icon is used on tabs where a favicon is not available.

Mozilla does not use this icon for regular tabs. To enable it, I will have to redo everything related to icons and media elements, which is not easy. For now, I will leave it as is. Then I will see if it is possible to do this without side effects.

I'll wait with this change for now. Mozilla is considering changing these icons in the near future.
