The MS Upgrade your account Ad is back, on top of the Inbox list. Highlighted in bold. Would be very nice to update the code to have this ad blocked again. Thanks very much!

I also now have this issue. It almost seems like Microsoft is adding these into the applications if it cannot reach outlookads.live.com. Even if they are deleted they reappear after the application is restarted.

I am also getting the "Upgrade Your Account ad", but for posterity here is my blocklist in my host file:
